Interested people can now simply fill up the UP Age certificate online at the website of the Department of Medical Health and Family Welfare (DoMH&FW).

How to apply for the Uttar Pradesh online application certificate?

  • Visit the official website of the Department of Medical Health and Family Welfare 
  • On the homepage, click the “Age certificate” option – this option is present under the Online Services section. After that click on “New Registration” below the Reset button.
  • Once the page loads, the UP age certificate issuance form will appear as shown here:
  • On this newly opened page, the candidates will have to fill in all necessary details and click on the “Register” button. Once that is done, the candidates will have to verify the OTP that is received on the phone number provided.
  • The candidate will then have to log in to open the “Citizen dashboard” which will appear as shown herewith:
  • Click on the “Issuance of age certificate” option which will open the Uttar Pradesh age certificate online application form
  • Finally, the applicant will have to fill the application form and click on the “Save” option to complete the application process.
